I used dragons mirror and decoy dragon activated effect and did not revive 5 headed dragon. I’m confused?

You are not confused. You just didn't read:

Must be Fusion Summoned, and cannot be Special Summoned by other ways.

Well, Cold Feet can be used in a deck with Mystical Refpanel, problem is, we don't have that card yet.
