It's up to the number of heads. Unless it is the only face-up monster, it doesn't have to destroy itself.

The Fusion, Gatling Dragon, says that it destroys AS MANY monsters as possible, equal to the number of heads. Desperado says it destroys UP TO the number of heads, which means you can choose to destroy 1, 2, or 3 monsters if you land 3 heads.
